With information gathered from the interview, medical health evaluation profile and the three day diet
food diary, the first steps of the individualized wellness plan will be outlined.
Discussion of possible root causes
Outline of personalized program
Outline of diet and lifestyle modifications
Personalized course of action plan
Possible root causes will be researched. In providing the information, it will give a well-rounded
outlook of health issues and will support an understanding of the physical, emotional, nutritional,
environmental and spiritual aspect of each component.
* It is imperative to recognize how present life style and behaviors can contribute to chronic conditions, and how changes in these behaviors can act in preventing disease and maintaining wellness.
From this cause and effect approach it will allow insight into what components of lifestyle and
environment are actively playing a role involving current health and wellness status. The information
reseached and shared will empower the client through the overview, and allow for an understanding of
how symptoms and problems evolve from lifestyle, environment, mind and body.
Sessions Three through Six: (Maintenance and Evaluation)
Follow up appointments are available - depending on your personal situation - to evaluate your progress, to address any issues that may arise during the course of the plan including your level of comfort with the components of the program and to ensure compliance.
